Clinic Policies:
All fees for services are the responsibility of the patient, payable in full at the time of the appointment.
Missed Appointments
Please have the courtesy to respect professional scheduling as someone else could benefit from the lost time slot of a missed appointment.
Patients will be charged for repeated missed appointments without 24 hours notice.
If the appointment was related to a voucher or promotion, then it will be void as a penalty for the missed appointment.
Cancelations or Re-Scheduling
Cancellation Policy:
If you are unable to attend your scheduled appointment, or if you must cancel your appointment for any reason – please provide the courtesy of at least 24 hours notice – so that your appointment time can be made available to someone else.
Appointments can be cancelled or rescheduled in the following ways: online 24/7 up to 2 hours prior to the appointment if you signed up with the online calendar; by sending a message through the website email booking@drjavitz.com; or by calling the office during regular business hours.
